Our NDIS provider services are tailored to meet the unique needs of individuals with disabilities, ensuring their well-being and quality of life.
0131 Specialist Disability Accommodation (SDA)
In some situations NDIS participants might require a higher level of living assistance. Specialist Disability Accommodation (SDA) is a range of housing designed for people with very high support needs.
The SDA Design Standard is a detailed set of requirements for all new specialist disability accommodation that covers 4 categories of SDA design:
- Improved liveability
- Robust construction
- Fully accessible
- High physical support
Not all people with disabilities are eligible for SDA. For a person to be eligible there are several factors that need to be considered by the NDIS
- If the NDIS participant has extremely high needs and needs to live in specialised homes.
- If the participant requires high levels of one-on-one or more support.
- If the participant is meeting the NDIS funding criteria.
0108 Assistance with Travel/Transport
The main purpose of this service is to increase participants’ independence. With this NDIS Support, the participants are able to access equipment and aids related to personal transportation or training in public transportation. The NDIS can also fund for Vehicle modifications for private vehicles such as cars or other vehicles participants or their carers own.
A participant will generally be able to access funding through the NDIS for transport assistance if the participant cannot use public transport without substantial difficulty due to their disability.
0125 Participate Community
Participate Community or Community Participation helps participants achieve goals relating to participating in community, social or recreational activities. These activities may be provided in a centre or in a community setting. This is a very important service for NDIS participants as it helps in promoting independence amongst the participants. The NDIS participants’ have the choice to choose the activities they want to get involved in. It allows them to connect with people who might have similar interests and goals.
Funding for Community Participation can be found both in core supports and capacity building supports. Assistance with Social and Community Participation is a support category that you can find in your NDIS plan, under Core Supports whereas Increased Social and Community Participation is a support category that you can find under Capacity Building Supports.
Our support workers can help you for the following:
- To attend personal development classes
- Go for an outing or a holiday
- Becoming a member of any social group
- Participating in community events or being part of a club
- Visiting the local library
- Going to the movies or for concerts
0107 Assist-Personal Activities
This support is available to NDIS participants who are unable to maintain personal hygiene and need assistance with performing personal tasks. Hope Help and Care (HHC) is a registered NDIS provider in the South Australia region, providing assistance with personal activities to NDIS participants.Some of the tasks our team at HHC can assist you with are:
- Maintaining personal hygiene, including showering, bathing, oral hygiene, dressing and grooming;
- Toileting, bladder and bowel management and menstrual care;
- Eating and drinking;
- Attending appointments;
- Use of aids and appliances, hearing and communication devices;
- Household tasks, such as cleaning, laundry, and shopping.
0115 Daily Tasks/Shared Living
At Hope Help and Care (HHC) we have professional and trained staff who have the right skills and abilities to meet the needs of individuals seeking care.. We can help NDIS participants with Assistance in Daily Tasks/ Shared Living.
Some of the services that we provide under Assistance with Daily Tasks:
- Assistance with cooking and meal preparation, including meal planning, grocery shopping
- Assistance with cleaning
- Assistance with laundry, including washing, drying, and folding clothes.
- Assistance with maintaining household tasks and performing routine maintenance tasks.
- Help with managing finances, including budgeting and paying bills.
0101 Accommodation/Tenancy
Assistance with Accommodation and Tenancy Obligations is defined by the NDIS as "support to guide or undertake activities to ensure the participant obtains/retains appropriate accommodation." This could include assisting with the application for a rental tenancy or carrying out tenancy requirements.
Under Accommodation/Tenancy we can help NDIS participants’ with:
- Assistance with private rental applications
- Budgeting Assistance for Housing
- Capacity Building to obtain long-term housing with increased independence
0111 Home Modification
Home modifications are changes to the structure, layout or fittings of a person’s home so they can continue to access and move about it safely, securely and independently. NDIS participants can access NDIS funding for this category If a suitably qualified Occupational Therapist has performed an assessment and recommended home modifications, or your primary house is not suitable to your requirements Hope Help and Care is dedicated to building lasting and profound connections with participants to ensure that your home fits your needs, regardless of complexity or scope.
0117 Development of Daily Living and Life Skills
Development of Daily Living and Life Skills is a service that promotes personal development, greater independence, and provides opportunities for increased social and community participation. At Hope Help and Care, we support NDIS participants to develop everyday life skills that increase independence and control. This service not only provides greater independence but helps participants build greater confidence and self-esteem. 0120 Household Tasks
Hygiene is one of the most important factors for any individual. For people with disabilities it may not be easy to do household chores alone and may require extra supervision. We at Hope Help and Care (HHC) can provide you assistance with Household Tasks to keep your environment clean and hygienic.
Some of the tasks we can help you with are:
- Garden Maintenance
- Grocery Shopping and Meal Cooking
- Taking people to doctor’s appointments
- House Cleaning including bathroom, kitchen, and dining area
- Vacuuming carpets and Mopping the house.
- Washing and ironing clothes.
- Changing of Bed linens and Towels.
0115 Short Term Accommodation including Respite
Respite services give an opportunity to people with disability to have some time away from their usual routine. To be included in your NDIS Plan, respite must be considered “reasonable and essential,” much like other supports under the NDIS. The NDIS takes into account if you are getting any support by informal relationships or family members.
We offer a range of respite services, including in-home respite, centre-based respite, and community-based respite.
Our in-home respite service allows our clients to receive care in the comfort of their own home. This service provides carers with the opportunity to take a break from their caring role while our staff take over the care responsibilities. We can provide short-term or longer-term care depending on the needs of our clients.
0127 Plan Management
Plan management is a service offered by the National Disability Insurance Scheme (NDIS) to assist NDIS participants in managing their NDIS funds. It involves the management and administration of the funding that an NDIS participant receives from the NDIS to cover the cost of their support and services.
There are three types of plan management options available to NDIS participants:
- Agency managed
- Self-managed
- Plan managed
We can provide NDIS participants with a range of services, including:
- Paying invoices and managing payments to service providers
- Keeping track of NDIS funding and expenditure
- Providing regular statements and reports on the participant's NDIS plan
- Advising on NDIS funding rules and regulations
- Providing assistance with budgeting and financial management.
Choosing plan management as an option allows NDIS participants to have more choice and control over the services and supports they receive, while still receiving assistance in managing the financial aspects of their NDIS plan.
